It's a judgement call.  A good throw, the guys out dead to rights, the fielder simply botches the play.  It's pretty rare on a steal attempt, but it's no different than say a play at the plate, where the ball is there and the catcher drops it.

Actually, you can assume a double play.  You don't charge an error on a bad throw to complete a double play, but if the fielder simply drops what should have been an out, you can certainly charge him an error.

BTW,

The official ruling on the caught stealing reads like this:

"When in the scorer’s judgment a runner attempting to steal is safe because of a muffed throw, the official scorer shall not credit a stolen base. The official scorer shall credit an assist to the fielder who made the throw, charge an error to the fielder who muffed the throw and charge the runner with “caught stealing.”

It's not about the front end or the back end, it's about whether or an out should have been made.  If a fielder simply muffs a throw that should get an out, he should be charged an error, regardless of whether or not any other outs are made on the play.
